Extended Access for Derby City South

 

Derby City South comprises of 9 GP surgeries across South Derbyshire, including:
Alvaston Medical Centre
Hollybrook Medical Centre
Parkfields Surgery
Chellaston and Melbourne Medical Centre
Willington Medical Centre
Haven Medical Centre
Wellbrook Medical Centre
Lister House Chellaston
Village Surgery

This group of practices has been successful in being awarded a pilot to roll out extended access to its combined 104,000 patient population. The pilot scheme will support the surgeries in providing additional appointments that are more accessible for all.

What will the service provide?
 

The pilot has been rolled out via a phased-approach with an overall aim of providing a multi-disciplinary approach to the treatment and care of our patients.

Three hub surgeries are now operating offering extended access appointments to patients from any of the 9 surgeries.

Weekday Hub operating 6.00pm - 8.00pm on weekdays

How do I access the service?

The provision of this service is only available to patients who are registered with one of the 9 surgeries within Derby City South.

Appointments are made via the patient’s own GP surgery and are available to patients who are unable to access appointments during normal surgery hours or when surgery appointments are fully booked.

What conditions can be treated?

The hubs are designed to treat patients with acute minor ailments who require a face-to-face clinical assessment in order to be treated appropriately.  It will be primarily Nurse Practitioner led, with the addition of a weekend 'Dressings' clinic at  the Haven hub on Saturdays.

Patients who cannot be seen at the hub:

  • Babies under 12 weeks old
  • Pregnancy-related problems
  • Chronic disease or long-term condition management that requires continuity of care
  • Severe mental health problems
  • Patients identified with a potentially life-threatening medical condition which requires referral to urgent or emergency care settings
  • Walk-in patients
